1. Assist-Life Stage, Transition

At Building Truss, we recognise transitioning through new stages of life, and how revisiting the past can sometimes present with challenges. Therefore, we are ready and equipped to assist our participants in all stages of their lives. This can include but is not limited to: - Leaving home or moving into a Supported Independent Living (SIL) accommodation; - Moving schools or finding the right learning education;
- Finding employment;
- Connecting with support networks, including peer supports;
- Life transition which can include, mentoring, development of skills and daily planning;
- Planning ongoing activities within the community to increase capacity;

Our approach to be successful in supporting our participants within these transition stages include developing personalised plans for each participant to understand their needs and goals, collaboration, and strategies for any new challenges that may present to assist participants in achieving their personalised goals. We can assist in accomplishing short term and long term based goals by assisting and tracking progress to live your best life.

2. Assist-Personal Activities

Our recruitment process will see exceptional and caring support workers assisting participants with personal care. This is an essential part of life and at Building Truss, we acknowledge and want to support participants to get the care they deserve and need. We will assist you with tasks such as grooming, showering, toileting, hygiene and maintaining cleanliness. Our aim is to work together with participants to develop skills where possible and implement self care skills routines.


3. Assist-Travel/Transport

At Building Truss, we believe everyone is equal. Therefore, we offer transport to assist participants to access transport using an appropriate and safe form of travel. Depending on the participants needs, our support workers can accompany the participant whether it be on public transport or via the support workers vehicle. If participants wish to have a goal to develop skills in using public transport, we encourage these new skills and are ready to assist with this. Building Truss have processes in place to ensure forms of travel are safe and we can confidently support the participant in accessing these Travel/Transport options. With the help of transport and travel options, this can open up a whole new level of independence for participants, and provides them the opportunity to fully participate in community, social activities, and personal tasks.

4. Development-Life Skills

Our focus is to support participants to develop new skills, leading to an independent and comfortable lifestyle. We do this by assisting in increasing their skill set, providing training, support, and overall increasing abilities to live life to the fullest whilst achieving personal goals. Some examples of these services are: • Support with shopping • Support with self care needs • Training, hygiene care and shower care • Planning and education with using public transport • Meal preparation/ education on planning meals • Developing routines, i.e prepared for appointments/employment • Community participation and peer groups Our personalised plans are aimed to work with participants and listening to their needs and goals. We will work towards the set goals in a positive and collaborative manner.

5. Participate Community

Our team value social participation and commitments, therefore accessing the community is a vital part of participants choices. By supporting participants to participate in community activities this will assist them in improving their communication skills, peer groups, social and emotional skills, confidence and mental wellbeing. Our support workers can assist with community activities and help with any opportunities for new interest in activities. We acknowledge wellbeing is a vital part to all our lives, therefore, we want to support everyone to equally enable connections and opportunities for new connections. We believe in creating safe settings and will have appropriate procedures in place to ensure participants feel safe at all times.

6. Therapeutic Supports

Our main goals are to support participants to improve their daily independence. We recognise that each participant will have different goals and needs therefore, we will tailor supports that ensure we are working on setting new goals, reviewing existing goals and providing achievable strategies. The areas we will cover are communication and language, community living, movement and mobility, and personal care. Our team will provide a safe and nurturing environment where you can feel heard and your wellbeing is at the centre of what we do. Social Work Services; Our current social worker is highly trained and experienced, and can provide a range of services, including NDIS planning, support coordination, and more. Some examples of the ways our NDIS-Registered social worker can help; - Personal and family mental health counselling services. Our social worker has extensive experience in providing individuals and families with counselling services.
- Information and referral to other NDIS services. Our social worker is knowledgeable in regards to what NDIS services are available and can help you find the right services for you. - Coordination with and facilitating linkage to relevant services. Once we’ve found and referred you to the right services, we can facilitate connecting you with the service provider and make sure you’re prepared. - Advocacy for individuals, families, and people experiencing social disadvantage. Our NDIS social worker can provide advocacy services for NDIS participants who may need extra support to access services.

7. Group/Centre Activities

At Building Truss, we recognise how important social connections are, therefore, based on their identified interests Building Truss support workers will connect participants with personal interests and hobbies that are important to them. While establishing friendships that come from shared interests and activities. These activities will assist with building life skills, confidence, peer relationships, cultural skills/interests and brings joy and happiness.
